Liberty Psychotherapy provides trauma-informed, community-based psychotherapy for individuals and families following acute mental health presentations, crisis intervention, or hospital admission.
This referral pathway is intended for hospital clinicians, acute care mental health teams, ambulance and mental health responders, and community health professionals seeking appropriate follow-up and continuity of care for their patients.
To ensure referrals are triaged appropriately, all professional referrals are accepted via the Online Referral Form on this page and you can expect your referral to be actioned within 24hrs of receipt.
The referral form allows you to provide relevant clinical context, presenting concerns, and referral rationale to support appropriate intake and engagement.
If you are unsure whether a referral is appropriate, you are welcome to submit a general professional referral form with brief details.
Individuals and families may also self-refer.

NDIS self-managed and plan-managed participants are accepted
